Official Rules & Terms and Conditions: August Giveaway

NO PURCHASE, TREATMENT, OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A PURCHASE OR APPOINTMENT WILL NOT INCREASE YOUR CHANCES OF WINNING.

1. Sponsor

The Sponsor of this giveaway is True North Chiropractic, located in Kaysville, Utah (“Sponsor”).

2. Eligibility

The Giveaway is open to legal residents of the United States residing in the State of Utah who are 18 years of age or older at the time of entry. Employees, owners, officers, and directors of True North Chiropractic, as well as their immediate family members (spouse, parents, siblings, and children) and household members, are not eligible to enter or win. Void where prohibited or restricted by law.

3. Promotion Period

The Giveaway begins on August 1, 2026, at 12:00 AM MST and ends on August 31, 2026, at 11:59 PM MST (the “Promotion Period”). Sponsor’s computer is the official time-keeping device for the Giveaway.

4. How to Earn Entries

Participants may earn entries through any combination of the following methods during the Promotion Period:

• In-Office Clinic Visit (1 Entry): Existing patients receive one (1) entry for each in-person clinic visit completed at True North Chiropractic during August.

• Google or Facebook Review (2 Entries): Established patients who have received care at True North Chiropractic prior to or during the Promotion Period will receive two (2) entries for posting a new review or updating an existing review on True North Chiropractic’s official Google Business Profile or Facebook Page. Reviews must reflect an honest, genuine opinion and experience; entry is awarded regardless of whether the review is positive, neutral, or critical.

• Video Testimonial (5 Entries): Established patients who have received care at True North Chiropractic prior to or during the Promotion Period will receive five (5) entries for recording a video testimonial about their experience and either posting it to Facebook (tagging True North Chiropractic or making the post public) or emailing/submitting the raw video file directly to the office.

• Patient Referral (10 Entries): Participants receive ten (10) entries when they refer a new patient to True North Chiropractic, provided the referred individual schedules and attends an initial appointment during August.

• Facebook Like/Follow: Participants may receive 1 entry for liking and/or following the True North Chiropractic & Wellness Center official Facebook page.

• Alternative Method of Entry / No Purchase Necessary (AMOE): To enter without visiting the clinic, making a purchase, leaving a review, or making a referral, individuals may visit the True North Chiropractic website, download and print the designated PDF “Health Quiz,” complete all fields legibly, and mail it in a stamped envelope to:

  Each fully completed and validly submitted mail-in Health Quiz receives one (1) entry into the drawing. Mail-in entries must be postmarked by August 31, 2026, and received no later than September 5, 2026. Limit one mail-in submission per outer envelope.

5. Winner Selection and Odds

On or about September 7, 2026, Sponsor will select one (1) potential winner in a random drawing from among all eligible entries received across all entry methods during the Promotion Period. The odds of winning depend on the total number of eligible entries received.

6. Prize and Approximate Retail Value (ARV)

One (1) winner will be selected. The winner may choose one (1) of the following three prize options:

1. Option A: A $50 Gift Card to a designated local restaurant (selected by Sponsor).

2. Option B: A $50 Account Credit toward future services at True North Chiropractic (subject to applicable clinical/legal regulations; non-transferable, no cash value, not applicable to services covered by federally funded health programs if restricted by law).

3. Option C: A $50 Visa Gift Card OR $50 Amazon Gift Card.

Total ARV of Prize: $50.00 USD.

Prizes are non-transferable, non-assignable, and no cash redemptions or substitutions will be made except at Sponsor’s sole discretion. All federal, state, and local taxes, if any, associated with acceptance or use of the prize are the sole responsibility of the winner.

7. Winner Notification & Claiming the Prize

The potential winner will be notified via phone, email, or direct social media message on or about September 8, 2026. The potential winner must respond and claim the prize within seven (7) business days of notification. If a potential winner cannot be contacted, fails to claim the prize within the allotted time, or fails to comply with these Official Rules, the prize will be forfeited and an alternate winner may be selected in a random drawing from remaining eligible entries.

8. Media, Privacy, and HIPAA Disclosure

By submitting a video testimonial, written review, or participating in the Giveaway, participants acknowledge and agree to the following:

• Video Testimonials & Reviews: Participants grant True North Chiropractic an irrevocable, royalty-free, perpetual, worldwide license to publish, display, reproduce, edit, and distribute their submitted video, written reviews, photos, name, and likeness for promotional, marketing, and advertising purposes across any media channel (including website, social media, and printed materials) without further compensation or notification.

• Voluntary Health Information Disclosure: Participants acknowledge that leaving a review or video testimonial voluntarily discloses patient status and personal health experience, and hereby waive any confidentiality claims under HIPAA or state privacy laws regarding information voluntarily shared in public forums or provided for promotional use.

9. Platform & Third-Party Disclaimers

This promotion is in no way sponsored, endorsed, administered by, or associated with Meta (Facebook), Google, Visa, Amazon, or any restaurant represented in the prize options. Entrants completely release Meta, Google, Visa, and Amazon from any and all liability connected with this Giveaway. Any questions, comments, or complaints regarding the Giveaway must be directed to True North Chiropractic, not to any third-party platform or vendor.

10. General Conditions & Limitation of Liability

Sponsor reserves the right to cancel, suspend, or modify the Giveaway if fraud, technical failures, human error, or any other factor impairs the integrity or proper functioning of the Giveaway, as determined by Sponsor in its sole discretion. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, illegible, misdirected, or postage-due entries or mail, which will be disqualified.

By entering, participants agree to release, discharge, and hold harmless True North Chiropractic, its officers, directors, employees, and agents from and against any claims, liabilities, losses, damages, or injuries of any kind arising out of or in connection with participation in the Giveaway or the receipt, possession, use, or misuse of any prize.

11. Disputes and Governing Law

This Giveaway and all issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules shall be governed by, and construed in accordance with, the laws of the State of Utah, without giving effect to any choice of law or conflict of law rules. Any legal proceedings arising out of this Giveaway shall be instituted exclusively in the state or federal courts located in Davis County, Utah.

12. Winner List

To request the name of the prize winner, send a self-addressed, stamped envelope to True North Chiropractic at the address listed above within 30 days following the end of the Promotion Period.
